Is the gym regularly cleaned, is their a weird smell upon entering? Are the bathrooms clean? Are you afraid to take a shower there?

Can you find equipment in a reasonable amount of time?
Equipment Condition*  
Is the equipment falling apart or are all machines in working order? Are most of the machines newer, or models from a museum on loan?

Is their other on-site services such as tanning, spa services, daycare? Many people enjoy having one place to workout and also get a massage or get their nails done.

Charged me for 5 months after canceled!

Joined Gold's Gym but moved away to NY in December to go to college and forgot to cancel before I left. Called them and they said I MUST appear in person! I said I was in NY and they said that's their policy!! Didn't come home to NH until May and had to drive all the way down. Charged me for the 5 months I was away and it stated on the form, I could have written them to cancel it!! Had called twice before and told had to come in person. Never trust them again. I would avoid from now on.
